hereby requests a shipping/transportation account with Reliable Trucking Services (“RELIABLE”) or an affiliate
company. CLIENT agrees that all invoices are due and payable in full within 30 calendar days of
invoice date.

Any invoice not paid within 30 days will incur a $50.00 late fee, plus interest at 1.5% per month (or
partial month) on the outstanding balance, or the maximum rate permitted by law, whichever is
greater. CLIENT agrees to reimburse RELIABLE for all costs of collection, including reasonable
attorney's fees and court costs, incurred in the enforcement of this Agreement. We report to credit
agencies.

CLIENT agrees to the RELIABLE terms and conditions available at www.reliabletruckingservices.com,
which are incorporated herein by reference.

Customer acknowledges that any LTL freight quotes are based on the details and services requested
at the time of booking. However, the use of additional services (Accessorials), including but not limited
to liftgate, residential delivery, inside pickup/delivery, re-consignment, or wait time, constitutes
automatic authorization for the associated charges. These services are often required by the carrier
to ensure safety or successful completion of the shipment and do not require prior written consent.
CLIENT is solely responsible for any and all accessorial fees.

All quotes issued by RELIABLE are for commercial freight only, between commercial addresses, and
are considered "dock to dock" unless driver assist or special handling is explicitly included in the
written quotation.

RELIABLE is not a "Carrier" at any time. If RELIABLE appears as "Carrier" on a Bill of Lading or any
other document, such designation is for shipper's convenience only and confers no carrier liability
upon RELIABLE.

Any claim for late delivery, damage, or shortage is a separate matter from the freight charges due.
Freight invoices must be paid regardless of any pending claim and may not be short-paid without
prior written authorization from RELIABLE.

An emailed, or electronic copy of a Bill of Lading, packing list, or similar shipment document
constitutes sufficient proof that RELIABLE rendered freight transportation services.
